CORI FORM

All in-school volunteers must have a current CORI form (criminal background check) completed within the past two years. To complete a CORI form, please see Heidi Walsh in the Oak Ridge school office

Please complete for all events and programs. Return to the Volunteer Coordinator box in the School office. Your summary will be available in the PTA filing cabinet for future chairpersons.
